Take care of constipation in children

Problems with defecation in children can sometimes cause very serious problems. The most important of these problems are constipation, fecal incontinence (encopresis) and soiling (soiling). Sometimes these problems are so great that even the doctor on the other hand may be in despair from time to time and have to think about what else I can do.

Constipation is a symptom rather than a diagnosis

” ConstipationAssociate Professor Salih Somuncu said, “Constipation should be considered in children, rather than a diagnosis. Constipation in children for a long time is a symptom of disease rather than a diagnosis. Among the reasons; Disruptions occurring in one or more of the normal defecation (defecation) phases, a bowel movement (motility) disorder involving the ends of the small intestine and the large intestine, and abnormalities arising from the content of the stool and itself. Certain criteria are absolutely necessary for the diagnosis of constipation. In these; the child has less than three defecations per week, the child has pain or excessive straining in one of every 4 defecations, the stool is hard and piecemeal in one of every 4 defecations,

Constipation may also occur when switching to complementary foods.

Assoc. Dr. Salih Somuncu said, “This is an important cause of constipation. An anal fissure that may accompany during this period may cause an even more vicious circle. Abnormalities in stool content also cause constipation. ” said.

“95% have malnutrition”

Somuncu said, “95% of constipations, the cause of which cannot be fully explained, are malnutrition, toilet habits, slow bowel movements, diseases of the nerves controlling the intestinal motility system, contraction of the internal sphincter, painful defecation and psychological factors. Especially some psychological factors and toilet training. It is thought to be important in this regard,” he said.

Diagnostic methods used to reveal the cause of constipation

Taking the patient’s history very well, detecting constipation and accompanying problems, onset time, examination of constipation
Physical examination: Many findings can be obtained by careful abdominal examination, anal-rectal examination, genital examination and neurological examination.
Laboratory studies: Plain abdominal X-ray, large bowel X-ray, ultrasonographic examination of the anal canal (endosonography), evaluation of defecation as X-ray (cinedaphacography), anorectal pressure studies (manometry), biopsy are the most commonly used methods.
Treatment in Constipation

In the treatment of chronic constipation, first of all, it is necessary to reveal the cause and to apply the treatment for the cause. An existing anatomical defect should be surgically corrected. If there is an internal disease that can cause constipation, it should be treated.

Causes of chronic constipation

  • Unknown (idiopathic)
  • Due to anatomical pathologies: anal fissure, anteriorly located anus, anal atresia (absence of anus) or anal stenosis (anus stenosis), enlarged large bowel syndrome.
  • Nervous system (neurogenic) causes: Congenital spinal cord diseases (such as spina bifida and meningomyelocele), spinal cord tumors, cerebral palsy (cerebral palsy), mental retardation, Hirschsprung’s disease, neurodevelopmental diseases of the intestinal system.
  • Hormonal and metabolic diseases: Under- or over-functioning of the thyroid gland (hypo-hyperthyroidism), certain kidney diseases, low calcium or potassium levels in the blood.
  • Depends on drugs: Anesthetic agents, epilepsy drugs, antidepressant drugs, etc.
  • Systemic diseases: Diabetes mellitus (diabetes mellitus), scleroderma.
